DOWNLOAD: Province of Davao Oriental Embarkation Form

We took the liberty to download and re-upload the Embarkation Form to make it easier for people to find and download it.

Effective June 1, 2020, the Province of Davao Oriental will open its border to neighboring provinces that are under the Modified General Community Quarantine (MGCQ).

Private and public commute will be allowed but preventive measures will still be implemented. Among which is the "No Embarkation Form, No Entry Policy".

It means all passengers coming in must fill out this form.

Passenger info in the EF will be encoded in a database for tracing purposes in case someone gets tested to be COVID-19 positive.
